  1. Ledger connection inadequate. Lag screws too small, wrong spacing, or no flashing. This is the most safety-critical connection on the entire deck. Inspectors scrutinize it heavily because ledger failure has caused fatal collapses.
  2. Missing or incomplete joist hanger nails. Every nail hole in every joist hanger must be filled with the correct nail. Common nails, drywall screws, and deck screws are not acceptable substitutes for joist hanger nails.
